Opening a physiotherapy practice in Nigeria involves a long list of decisions: location, equipment, registration with the Medical Rehabilitation Therapists Board, and if you are in Lagos, registration with HEFAMAA. Somewhere on that list, usually near the bottom, sits the question of what software the practice will actually run on. It deserves to be much higher up, because the systems you choose in the first month tend to stay in place for years, and switching later is far harder than starting correctly.
This checklist covers the software decisions worth making before you open, in the order they tend to matter most for a new practice starting out in Nigeria.
1. A Proper Patient Record System From Day One
It is tempting, when you have one or two patients a week, to keep records in a notebook or a simple spreadsheet. The problem is that this habit does not scale, and by the time it becomes a problem, you have months or years of records trapped in a format that is painful to migrate. Starting with a structured electronic health record system, even while your patient list is small, means every record from your very first patient is searchable, backed up, and ready to scale with you.
This also matters for HEFAMAA registration if you are operating in Lagos, where digital record keeping is explicitly listed as a preferred standard for private clinics.
2. Online Appointment Scheduling
A new practice depends heavily on making it as easy as possible for a first time patient to book. A booking link that shows real availability removes the friction of a phone call during a session, or a WhatsApp message that gets answered hours later. For a practice still building its patient base, that friction is the difference between a booked patient and one who simply tries the next clinic on their list.
3. Structured SOAP Documentation
New practices often underestimate how much time documentation takes once patient volume grows. Establishing a structured SOAP note format from the start, covering subjective findings, objective measures, assessment, and plan, means your documentation habits are already correct by the time volume increases, rather than needing to be fixed under pressure later.
4. Invoicing Built Into the Same System
With the overwhelming majority of Nigerian patients paying directly out of pocket, a new practice cannot afford loose invoicing habits. Every session should generate a trackable invoice automatically from the first patient onward, so outstanding balances are visible from day one rather than becoming an untracked problem once volume grows.
5. Outcome Measures From the First Patient
Standardised outcome measures, such as the PSFS, ODI, NDI, or DASH depending on presentation, give a new practice something powerful: proof of results from the very beginning. A new clinic without an established reputation benefits enormously from being able to show a patient concrete, measurable progress rather than relying only on reassurance.
6. A Way to Deliver Home Exercise Programmes
New practices often serve a wider catchment area than they expect, including patients who cannot always make it in for every session. A structured way to build and deliver a home exercise programme, one that can be tracked and adjusted, keeps those patients engaged in their care between visits rather than losing contact with the practice entirely.
7. Telerehabilitation Capability
Even if you do not expect to use it often in your first months, having telerehabilitation available from the start means you can accept patients who are travelling, recovering post surgery, or based outside your immediate area, without turning them away simply because there is no structured way to treat them remotely.
The systems you choose in your first month of practice are the ones you will still be using, or painfully migrating away from, three years later.
Why Building This on One Platform Matters
It is possible to piece together separate tools for records, scheduling, invoicing, and telerehab, but a new practice rarely has the time or budget to manage several disconnected systems. A single platform that combines these functions means a booking automatically becomes a patient record, which flows into a SOAP note, which connects to an invoice, without manual re-entry at every step. For a solo practitioner or a small new team, that saved time is often the difference between focusing on patients and drowning in admin.
Getting Started
You do not need every feature running perfectly before your first patient walks in. Most new practices start with records, scheduling, and invoicing in place, then add outcome tracking and telerehab as the practice grows. What matters is starting with a system built for how physiotherapists actually work, rather than adapting a generic tool later once bad habits and scattered records have already accumulated.
PhysioFlow was built specifically for this stage, combining EHR, scheduling, invoicing, outcome measures, and telerehab in one platform designed for physiotherapy practices from their very first patient.

The clinics that grow smoothly in their first few years are rarely the ones with the most equipment or the biggest space. They are usually the ones that got their systems right before their first patient ever walked through the door.
